In order to complete the arrangements, please follow the steps listed below:


  1. Fill out and complete the Authorization Form.
  2. Obtain a copy of the Death Certificate. This is a requirement of the United States Coast Guard.
  3. Obtain a copy of the Release Form from the crematory (this may be sent by the crematory with the cremains).
  4. Captain's Accompanied Service - Cremated remains and all required forms should be brought to boat the day of trip or mailed in advance.             
  5. Captain's Farewell Service - Cremated remains and all required forms should be made available either by mail or local pickup.


Payments:

May be made by cash, cashier’s check, USPS money order or personal check. Make checks payable to: New Choice Charters Inc.


Shipping:

The United States Postal Service will ship cremated remains.  Cremated remains should remain in the plastic container in which you received them.  Place the original box inside another box/container.  All shipping must be in accordance with

USPS Publication 139.  Should you have any questions in regards to shipping, please contact your local postmaster.
