In order to complete the arrangements, please follow the steps listed below:


  1. Fill out and complete the Authorization Form.
  2. Obtain a copy of the Death Certificate. This is a requirement of the United States Coast Guard.
  3. Obtain a copy of the Release Form from the crematory (this may be sent by the crematory with the remains).
  4. Ship the cremated remains and all required forms to: New Choice Burials – 2060 NE 124 ST – North Miami, FL 33181-2617


Payments:

May be made by cashier’s check, USPS money order or personal check. Make checks payable to: New Choice Charters Inc.


Shipping:

The United States Postal Service will ship cremated remains.  Cremated remains must remain in the plastic/cardboard box in which you received them.  Place the original box inside another box/container including all the necessary forms and fees.  Remains must be shipped Registered Mail with Return Receipt Requested.  All shipping must be in accordance with  USPS Publication 52.  Should you have any questions in regards to shipping, please contact your local postmaster.

 If you are local with 10 miles of our mailing address, we will pick-up at no extra cost or you can choose to deliver to us.
